Anomalies — guide
How to report unsafe conditions, unsafe acts and safe observations, and how they are followed up.
Report on the spot
Choose the kind and category, write one line about what you saw and tap the risk. The date, time and your name are filled in. It also works without internet.
Act immediately
If there is danger, make the area safe first and write what you did. Tick “Work was stopped” when the job was stopped.
Add photos
Photos are stored privately and only your company can see them. Without a connection, add them later on the anomaly page.
Corrective actions
When the cause needs more than the immediate action, add corrective actions with a responsible person and a due date. They also appear in the CAPA module.
Close
An anomaly can be closed when no action is still open. Without actions, the recorded immediate action is enough. A safe observation is closed automatically.
Quick risk (company 5×5 matrix)
Tap the cell where likelihood (1 very unlikely … 5 very likely) meets severity (1 very low … 5 severe). Six levels: very low, low, medium, high, very high, very severe. From «high» upwards the hazard must be controlled before work continues; «very severe» means stop the work.
Blame-free reporting
Reports are used to find and remove causes. Naming the person in an unsafe act is optional and the name is visible only to the owner and managers.
